Jay Perkins, 56, died at WFH- All Saints on Monday, December 13, 2010.

He was born in Minneapolis, MN on February 6, 1954, the son of Ralph and Verna (nee: Schmidt) Perkins. They both preceded him in death.

Jay served his country with the U.S. Marine Corps during the Vietnam War.  He was employed by S.C. Johnson.

Jay is survived by his brother, Kent Perkins of Pardeeville, WI.

There will not be a service held.  If you would like to contribute a memorial in Jay's name, Second Harvest Foodbank of Southern Wisconsin has been suggested.
